I came across this twice. I am only in the beginning of the book and noticed it once and was just prompted to read the Whalestoe Letters and saw this again.
Any significance?

I'm pretty sure it's an astrology pun. Pisces represents the 12th astrological house (the house of self-undoing) which could reference prison, hospital, etc.
